The Yavapai Lions Club offers eyeglasses, or related  services to individuals needing low- income  economic assistance for eye exams, and the purchase of eyeglasses. If you are a resident of Yavapai County and need assistance with eye or hearing care, please fill out the Application for Assistance Form below.
