To  complete an Application for Membership using a PDF fillable form online below.

When you click on the “Fillable Online Form” below a form will display on your monitor where you can complete the various highlighted fields.

Once completed, download the form to a folder on your computer.  Normally, the form will be saved in your download folder, but not always, so you must remember where it is located for future reference.

Next do one of the following:

(1) SUBMIT BY EMAIL: After completing the application, send an email to rhunonelson@gmail.com with a printed copy of the form, and any supporting documents .

(2) SUBMIT BY MAIL: After completing the application, mail it, and any supporting documents to Prescott Noon Lions Club, P.O. Box 2215, Prescott, AZ  86302-2215
