Prescott’s Lion Clubs offers economic assistance for eye exams, the purchase of eyeglasses, and related services to low-income beneficiaries.
Residents of Yavapai County requiring financial assistance with eye care, or hearing services, are encouraged to fill out an Application for Assistance Form using one of the following options.

Once completed, download the completed form, then print the form, and sign and date it.
Finally, mail it with the requested support documents to the address listed on top of the form.
